This transcription is based on a source (in Spanish Tablature) I found in the Gemeentemuseum from The Hague for five-course guitar.
In the Instruccion de Música sobre la Guitarra Espanola (1674) Sanz gives some very detailed information regarding tunings.
My five-course guitar has been specially stringed for the music of Robert de Visée and Francesco Corbetta and has a double E4 (mostly left at that time, regarding tuning problems with the gutstrings).
It is tuned A3A3-D3D4-G3G3-B3B3-E4E4 (but A3A4 was also in use in the time of Sanz).
As you can see the octave string D4 has been placed on the opposite side when regarded with a "normal "12 string.
I didn´t notate hammer-ons and pull-off when they where not notated in the Spanish Tablature (reversed when regarded with this tablature-system). I notated as less rests as possible to emphasize the special character of this guitar (Please note that most published versions are an interpretation of the tablature and reworked for a normal 6-string Classical Guitar).
